choices…for a Healthy Future

Teva Canada offers a comprehensive and competitive benefit plan designed to provide you and your family with protection, choice, flexibility and opportunities to build retirement savings.

The choices Benefit Plan, including health, dental, disability, wellness programs, life insurance and retirement plans, is available on the first day of employment to all eligible Teva Canada employees. And, when you select a Teva drug, you do not pay any portion of the cost. Reimbursement is 100% with no out-of-pocket cost to you. To help you find a pharmacy that carries the Teva drug you need, use the “Pharmacy Search” functionality available on your Manulife group benefits home page.

For more details, see the 2025 Benefits Guide.

Health & Wellbeing

With the choices Benefit Plan, you and your family can select from three Health and Dental Plan options. Your elections will be effective beginning on January 1 (or your date of hire) through December 31 each year.

If you experience an eligible life event during the year (e.g., marriage, new baby, etc.), you can add or remove dependents as necessary.

For Health and Dental Plan options, you can choose to cover yourself or a family through Manulife Financial Group Benefits. Each choices Benefit Plan includes inclusive coverage for infertility treatment and gender-affirmation treatment, up to lifetime maximums. You can select from:

  • choice 1: Teva pays the full costs associated with the Health and Dental Plan, contributes to a Healthcare Spending Account (HCSA) and provides the ability to direct up to 50% of your HCSA to a Fitness Account. Prescription drug coverage is automatically provided.
  • choice 2: You pay a portion of the costs associated with the Health and Dental Plan through regular payroll deductions. Teva contributes to a HCSA and provides the ability to direct up to 50% of your HCSA to a Fitness Account. Prescription drug coverage is automatically provided.
  • choice 3: You pay a portion of the costs associated with the Health and Dental Plan through regular payroll deductions. You do not have access to a HCSA, but Teva contributes $400 to a Fitness Account. Prescription drug coverage is automatically provided.

Unmarried, dependent children under age 21, or up to age 25 for those attending university or a similar educational institution on a full-time basis, are eligible for Health and Dental Plan coverage. Verification of a dependent student must be completed each August for the upcoming school year. If your dependent child is not eligible for continued coverage as described, you must update your choices account and remove the dependent from active coverage. For more information, please see the Student Verification flyer.

To coordinate claims with another benefits plan (e.g., your spouse’s plan), it is essential that you notify Manulife of any secondary coverage. To make updates to your coordination of benefits, follow the online instructions. If you have difficulty making changes online, contact Manulife Group Benefits at (800) 268-6195.

For more details about each Health and Dental Plan option, see the Overview of Health and Dental Coverage. To access claims information and forms, visit manulife.ca and be prepared with our Health and Dental Plan policy number (86397).

When you enroll in the choices Benefits Plan, you automatically receive prescription drug coverage based on the plan you elect.

An informational pay direct drug card from Manulife Group Benefits will be provided to you, and you can also print a copy of the card from your Manulife homepage. You can use the card to have your eligible prescription drug claims reimbursed automatically at any pharmacy that uses the pay direct system. All you pay is your share of the cost (if any).

Company-paid Short Term Disability (STD) benefits provide a continuation of pay while you are unable to work due to non-occupational illness or injury.

The STD Plan can continue a portion of your salary for up to 26 weeks. This benefit is equal to 75% of base weekly earnings, up to a maximum of $3,500 per week.

Benefits begin on the first day of disability due to an accident, first day of disability due to hospitalization or on the sixth day (or first workday following one work week) for disability due to illness.

If you are disabled, and your disability continues beyond the 26 weeks covered by the Short Term Disability Plan, the Long Term Disability (LTD) Plan may provide you with financial security until you return to work (provided you are deemed disabled by the insurance company).

You pay for LTD coverage through payroll contributions, and the benefit, when paid by the Insurer, will be paid on a tax-free basis. Benefits may continue until you recover, you die or you retire, whichever comes first.

You have three LTD options to choose from (and the cost of coverage will depend on your selection and salary):

Plans:

choice 1

choice 2

choice 3

45% of salary up to a maximum of $11,000 per month*

50% of salary up to a maximum of $11,000 per month*

60% of salary up to a maximum of $11,000 per month*

* to $16,000/month upon evidence of good health

Examples: Cost of coverage

The cost of coverage is based on salary (annual base salary and target bonus). Premium rates are calculated per $100 of LTD benefit on a monthly basis. These cost of coverage examples feature premium rates effective January 1, 2025.

The premium rates for 2025 are: choice 1: $1.515; choice 2: $1.651; choice 3: $1.856

choice 1
($80,000 salary)

choice 2
($50,000 salary)

choice 3
($65,000 salary)

Step 1: $80,000 / 12 months = $6,666.67

Step 2: $6,666.67 x 45% = $3,000

Step 3: $3,000 x $1.515 / $100 = $45.45

Step 4: $45.45 x 12 months / 26 pays = $20.97 per pay

Step 1: $50,000 / 12 months = $4,166.67

Step 2: $4,166.67 x 50% = $2,083.34

Step 3: $2,083.34 x $1.651 / $100 = $34.40

Step 4: $34.40 x 12 months / 26 pays = $15.87 per pay

Step 1: $65,000 / 12 months = $5,416.67

Step 2: $5,416.67 x 60% = $3,250

Step 3: $3,250 x $1.856 / $100 = $60.32

Step 4: $60.32 x 12 months / 26 pays = $27.84 per pay

If you want to increase the amount of LTD coverage you have (following your initial choice), you will be required to complete an Evidence of Insurability form. When applying for LTD you must be actively at work in order for your application to be valid. If you do not complete the EOI form when required or if you are not approved by the insurer, you will not receive the increased coverage you requested. You will be notified when your increased coverage is approved.

To calculate your LTD premium rates, understand the criteria for qualifying for LTD benefits and view additional plan details, please see the 2025 Benefits Guide.
